    AUTO PAY AUTHORIZATION
    FOR YOUR MICHIGAN CENTRAL BROADBAND ACCOUNT

    1. Print out the Auto Pay authorization form by clicking on the "PRINT AUTO PAY FORM" button below.
    2. Complete and sign the form.
    3. Attach a voided check of the bank checking account or deposit slip of the bank savings account to be accessed for automatic payment of your phone bill.

    4. Mail the signed form and attachment to:

    5. We will send you a written confirmation advising the date of your first scheduled auto pay deduction.
